Fees and Policies

  1. Forms. The fee for school, camp, and sports participation forms NOT completed at the time of a physical/ check-up is $25 per form. Please give us 3-5 days turnaround time to have forms completed.
  2. After Hours Triage. After hour phone calls will result in a $20 charge. Calls resulting in being sent to the Emergency Department are exempt from this fee.
  3. Medical Records. The fee for medical records is $25 per patient (additional patient fee of $15). Please complete the medical request form available on our website and email us at [email protected].
  4. Cancellations and No-Shows. Scheduled appointments must be cancelled 24 hours in advance. There will be a $75 charge for any no shows or cancellations less than 24 hours in advance, this includes same day appointments. Your insurance company is NOT responsible for this charge, and you will be asked to pay this charge before scheduling the next visit.
  5. Proof of Insurance. A copy of the insurance card is REQUIRED at every visit. It must be emailed or provided to our staff before or at the time of the visit. If you do not provide a copy of the insurance card and we are unable to verify your child's insurance, you will be required to submit a NON-Refundable administration fee of $50 before the visit.
  6. Copays. Copays are required to be paid at the time of visit. This is an agreement between you and your insurance company. If you do not pay at the time of the visit a $10 administration fee will be added to the copay.
  7.  Sports Physicals. The fee for Sports Physicals is $75. This fee is due at the time of the appointment. Sports Physicals are not covered by insurance.

The Kidz Docs has implemented the following polices:

  1. New Patients must arrive 15 minutes before the appointment time. Please bring a copy of your insurance card to all visits. All newborns should be added to parents' insurance before the office visit. If they are not added, we will ask that you call and add them and provide a reference number.
  2. Sick Visits. Please schedule sick visits by calling the office and talking to our triage nurses. All children who need to be seen MUST have a sick visit scheduled (including siblings of patients scheduled). If you have questions, our nurses are trained in pediatrics and may give advice for home care or may recommend scheduling an appointment.
  3. Well Child Checks/ Physicals. You may schedule physicals/ checkups by calling the front desk or sending a portal message to our front desk staff. Summers are extremely busy; please call early in the summer/ spring to schedule the physical. Please bring any school or camp forms to these visits. Any forms not completed at time of visit will incur a charge as above.
  4. After Hours Care. We provide After Hours care for URGENT questions that cannot wait to be addressed until the morning. The after-hours line does not accept medication refill requests. Please send a message through the portal for refills or call during office hours. See fees above.
  5. Refills. Due to DEA regulations, all controlled substances including stimulants require follow up visits every 3 months and require one in person visit per year. We require that a follow-up be scheduled every 3 months. For monthly refills, please request refills at least ONE WEEK before they are needed. Some chronic problems will require a follow up appointment every 6 months (ie asthma, eczema, etc). This is not only an insurance requirement but also good medical care. We may ask that you schedule the follow up before the refill is placed.
  6. Insurance and Billing questions. Please call Trusted Doctors for any billing or insurance questions at (703) 322-0245.
  7. Vaccination Policy. Please see our vaccination policy on our website TheKidzDocs.com. Although we do allow alternate schedules, we recommend following the CDC and AAP recommendations. Required vaccines are also listed on our website.
  8. Photography and Videotaping. Staff and providers retain the right NOT to be videotaped or photographed. Please ask before videotaping or photographing. If you are asked to turn off video camera, please do so immediately. No videotaping or photography of procedures or vaccines is allowed. Due to HIPAA, no photography or videography of other patients is allowed. No photography or videography is allowed in common areas like waiting rooms or hallways.
